Creating purposeful partnerships with integrated community teams, is an effective way to collaboratively co-design models of care which support seamless patient transitions while reducing administrative burden and hospital congestion unique to your site. Alignment of vision and goals allows hospitals and integrated community teams to rapidly respond to changing needs and new opportunities in the healthcare space.
Through community partnerships, we will share examples of unique programs of care which support bundled funding models, frail elderly patients who are at risk of becoming ALC and ALC transitional rehab models. Performance metrics show significantly reduced hospital length of stay, reduced hospital readmission and reduced unnecessary emergency department visits but most importantly show more positive patient experience and outcomes.

HealthcareRounds cancellation requests must be submitted in writing to firstname.lastname@example.org and received no later than March 21, 2019. There will be no refunds for cancellation requests received after March 21, 2019.
No-shows occur when individuals register but do not attend the conference and are not eligible for a refund. The HealthcareRounds Planning Committee reserves the right to make changes in programs and speakers, or to cancel the HealthcareRounds if registration targets are not met or when conditions beyond its control prevail. If the HealthcareRounds is not held for any reason, Longwoods Publishing’s liability is limited to the refund of the registration fee only.
If you are unable to attend the HealthcareRounds, you are welcome to send a colleague in your place. There is no fee to make this change. Substitution requests should be sent to email@example.com.
